Job Duties:

• Provide direct patient care, including assistance with activities of daily living (ADLs), such as bathing, dressing, and feeding.
• Monitor patients' vital signs, observe for changes in condition, and report any concerns to the registered nurse or healthcare provider.
• Administer medications, injections, and treatments as prescribed by healthcare providers, ensuring accuracy, safety, and adherence to medication administration protocols.
• Document medication administration, dosage, and patient response in electronic medical records (EMR) or paper charts.
• Assist with wound care procedures, including cleaning, dressing changes, and applying topical medications, under the direction of the registered nurse or wound care specialist.
• Educate patients and their families about their medical conditions, treatment plans, and self-care techniques, including medication management, dietary restrictions, and home safety measures.
• Conduct basic patient assessments, such as measuring height, weight, and vital signs, and document findings accurately in patient records.
• Maintain comprehensive and up-to-date documentation of nursing care provided, including nursing notes, progress reports, and incident reports.
• Collaborate with interdisciplinary healthcare team members, including registered nurses, physicians, therapists, and social workers, to ensure coordinated and holistic patient care.
• Communicate effectively with patients, families, and caregivers, addressing their concerns, answering questions, and providing emotional support.

Job Requirements:

• OR LPN State License
• BLS – AHA
